Training Course Privacy & Policy

• General Enrollment and Attendance Policy

​​1. Legal Identification: Participants must register using their full legal name exactly as it appears on a valid government-issued identification document.

2. Accuracy of Information: It is the participant’s sole responsibility to verify that all information provided during registration is accurate. Typographical or data errors that require the reissuance of a physical or digital credential may result in an additional administrative fee. 

 

3. Valid Identification: The following current identification documents are accepted:

1. U.S. state or municipal-issued identification

2. Passport

3. National identification card from the participant’s country of original

4. Consular identification or consular registration card

 

4. Training Format: All training programs are conducted exclusively in person. Full and punctual attendance is required to be eligible for certification.

5.Late Arrival Policy: Participants may enter the class up to a maximum of 15 minutes after the official course start time. After this grace period, the participant will be required to reschedule the class and applicable fees will apply.

 

6. Prohibited Substances: Individuals under the influence of alcohol or any substance that may impair their physical or mental condition will not be admitted under any circumstances.

7. Third-Party Acceptance: Participants and contractors are responsible for confirming with their employer or applicable agencies whether the certificate issued meets their specific requirements. Zorvak LLC does not guarantee acceptance of its documentation by third parties.

8. Certificate Status: The provisional Certificate of Completion issued by the Trainer at the end of the course confirms the participant’s attendance and successful completion of the training. Under no circumstances does it replace or substitute the official OSHA card.

9. OSHA Card Collection: Official OSHA training cards must be claimed or collected by the participant within a maximum of 15 calendar days after notification that the card is available.

10. Federal Document: OSHA cards are official, individual, and non-transferable federal documents. Misuse, alteration, or falsification may result in immediate cancellation of the document and potential legal consequences.

11. Program Changes: The OSHA-authorized Trainer reserves the right to modify course schedules, dates, or locations due to logistical reasons or circumstances beyond their control. Participants will be notified in advance whenever possible.

Payment Policy and Administrative Terms

1. Advance Payment: The full course fee must be paid in full before the official start of the class. Participants with outstanding balances will not be granted access to the classroom or training materials.

2. Accepted Payment Methods:

1.Cash (All cash payments must be accompanied by an official receipt issued at the time of payment.)

2. Zelle

3. Credit/Debit Cards (Visa / Mastercard)

4. Personal checks are not accepted.

3. Company Checks: Business or company checks may require up to 5 business days for verification before training materials or certifications are provided.

4. Returned Checks: Any check returned due to insufficient funds or for any other reason will incur a $30 administrative fee.

 

5. Refunds: Payments are non-refundable once the class has begun. Before the course starts, participants may request to reschedule their training in accordance with the applicable policies.

6. Included Fees: Standard course fees include in-person instruction, training support materials, and processing of basic documentation.

Administrative Fee Summary

Upon registration, participants acknowledge and agree to the following additional non-refundable fees:

Rescheduling / Voluntary Date Change: $50

(Except in cases of medical emergencies or properly documented emergencies.)

Replacement of Lost or Damaged OSHA Card: $40

Returned Check Fee: $30

All fees are non-refundable unless otherwise stated in writing by the administration.

 

Diversity, Inclusion, and Non-Discrimination Policy

1. We promote an environment of respect, inclusion, and equity in all of our training activities.

2. Scope: This policy applies to all administrative staff, instructors, students, and subcontractors involved in our training programs.

 

3. Zero Discrimination: Discrimination based on any of the following is strictly prohibited:

  • Gender

  • Age

  • Culture

  • Skin color

  • Religion

  • Ethnicity

  • Sexual orientation

  • Nationality

  • Military or immigration status

  • Disability

  • Housing status

  • Language

  • Political ideology

  • Union affiliation

  • Personal appearance      

  • 4. Equal Access: We ensure equal access to all of our training programs, courses, and events.

 

Privacy and Records Protection Policy

1. Legal Compliance: We protect participants’ information in accordance with applicable privacy laws in the United States.

 

2. User Rights: Participants have the right to review their personal records and request corrections to their information.

 

3. Records and Formats: This protection applies to both physical and electronic records, including information collected through email, web forms, or digital platforms.

4. Transparency: The privacy notice is made available to participants during the registration process.

 

5. Security: Reasonable administrative, technical, and organizational measures are implemented to prevent the loss, theft, or unauthorized access to information.

6. Data Retention: In accordance with applicable guidelines, digital training records will be maintained confidentially for a minimum period of 5 to 6 years from the completion of the course.

7. Communications: All official communications regarding participant information, grades, or certificates must be conducted by email in order to maintain an auditable record.

8. Staff Responsibility: We ensure that all personnel who handle participant information comply with applicable data protection measures and act responsibly.

Records Disclosure Policy

1. Confidentiality: Unauthorized disclosure of participant records is prohibited, including billing, registration, grades, or attendance information.

2. Commercial Use: Participants’ personal information will not be shared with third parties for commercial purposes under any circumstances.

3. Authorization: Participants may authorize the release of their records to a third party, such as an employer, only through explicit written and signed consent.

Cancellation and Rescheduling Policy

1. Written Requests: All requests for cancellations, date changes, or rescheduling must be submitted in writing via email info@zorvak.com

 2. Telephone Contact: Participants may contact us by phone to receive general information; however, telephone communication does not replace the requirement for written notification.

 3. No-Show / Absence: Failure to attend without prior notice or deliberately abandoning the training will be considered an “incomplete course.” This will result in the forfeiture of the full payment, with no right to a refund or certification.

 

4. Force Majeure: In cases of medical emergencies or other force majeure circumstances that are properly justified and supported by verifiable documentation, the instructor may evaluate the possibility of rescheduling the course at no additional cost.

Class Cancellation Due to Conditions

1. Monitoring:The trainer continuously monitors conditions that may interfere with or disrupt the training

 

2. Company Cancellation: Classes may be canceled or postponed when necessary due to operational circumstances or force majeure events.

 

3. Notification: In such cases, participants will be notified promptly by email, telephone, or text message.

 

4. Guarantee:If the trainer cancels the class, participants will be offered new dates in a timely manner or the option to keep their payment as a credit toward a future course

 

Drug-, Alcohol-, and Tobacco-Free Policy

1. Zero Tolerance: We maintain a safe, healthy, and professional learning environment. The use, consumption, sale, or possession of drugs, alcohol, or tobacco products—including electronic cigarettes and vaping products—is strictly prohibited on the course premises.

2. Expulsion: Failure to comply with this policy will result in immediate removal from the class, loss of certification eligibility, and forfeiture of the payment made for the course.

Intellectual Property and Conduct

​1. Copyright: All educational materials, presentations, handouts, and examinations provided are protected by copyright. Reproduction, distribution, or commercial use without prior written authorization from Zorvak LLC is strictly prohibited.

 

2. Expected Conduct: Participants are expected to conduct themselves in a respectful, professional, and cooperative manner. Active participation is required in order to receive credit for the training hours.

 

3. Use of Technology: The use of cell phones or other electronic devices is prohibited during instruction and examinations, except in cases of medical emergencies previously reported to the instructor or an emergency involving an immediate family member.

 

OSHA Certification Validity Policy

1. Federal Regulation: OSHA 10-Hour and 30-Hour course cards do not expire under U.S. federal law.

 

2. Contractor Requirements: However, many employers, unions, general contractors, or specific projects may require workers to have completed or updated the course within the past 5 years.

3. Prior Verification: Participants are responsible for verifying the policies and requirements of their workplace before enrolling.
